
Do we have dental cards for employees?
Yes. Most dentists have a standard claim form that they may mail to the
Allied address. You may also pick up a dental claim form from your employer, Treasurer's office or download from the
"Forms" section of this website.
Do we have a dental network?
No. You may select your own provider. There is no dental network at this time.
When should I schedule my preventive cleanings?
You are covered for two Class I services per 12 months. Your appointments should
be scheduled at least 12 months later than the service before your last service.
For example, if you had a cleaning on January 25, 2000 and June 20, 2000. You
should schedule your visit 12 months after January 25, 2000 visit which would be
January 26, 2001.

My proposed dental treatment is over $1000. The dental office is requiring
that I pay 50% of the cost before they will remove my wisdom teeth. Is that an
unusual request?
Some dental providers require partial payment up front to protect themselves
from not being paid. In order to protect yourself, you should request a
pretreatment review when charges for a proposed dental treatment are over $500
for you or your dependent.
Ask your dentist to submit a claim form showing the proposed services and fees.
This "Predetermination" of Benefits will take into account Usual and
Customary, Benefit Limitations and alternate procedures. You will know before
any work begins just what your benefits should be. You and your dentist can then
discuss the proposed procedures and the benefits you will receive - before you
have incurred any charges.
